Panel 1: We start off with John and Jean working on a patient; as he does so, John thought-bubbles "Ted is a miserable [expletive deleted]....and to think I used to like him!!"
Panel 2: As he places something in Jean's hand, he thought-bubbles that Ted is the most pompous, selfish, inconsiderate...
Panel 3: Jean interrupts his mental rant by asking him if something is bothering him; he lies, says no and asks why she's asking.
Panel 4: Jean points out that he's just stuffed two rolls of cotton up Mr Morberg's nose.
Summary: When you realize that John doesn't have his facts straight and it's not really his business what Ted does AND that the whole thing was touched off by his needing somewhere to go on vacation, it's sort of hard to see why he's so angry that he can't keep his mind on his job. It's also sort of hard to see why people would go to a dentist this inept.
