Panel 1: We find ourselves watching John and Elly drive up to a community centre for Lamaze classes. When she states that she's glad he doesn't mind coming with her, he says "Not a bit" because we can't have him look like a witless fool husband if he's not present.
Panel 2: As they walk to the room the classes are in, she says that even though they've been through this before, it's good to have a refresher course. John merely says "Uh-huh" because he doesn't want to go down the rabbit hole of pointing out how poor Elly's memory is.
Panel 3: We hint broadly as to the punchline when she says that even thought it's been ten years or so since they had Liz, it shouldn't take long for everything to come back.
Panel 4: It would appear that "knowing when a woman is pregnant" has yet to come back for John when he leaves Elly gobsmacked by asking if she's sure this is the right room despite being surrounded by women in the second or third trimester. This is owing to Lynn's need to hammer us over the head that John is incapable of understanding pregnancy.
Summary: Lynn sees John's cluelessness and raises John's being vaguely unsettled by 'intruding' into a place where he 'doesn't' belong. This contrasts with Elly's superior physical awkwardness to make him look like an even more clueless dipstick.
