Panel 1: We find Elly eating an apple and raiding the fridge and thought-bubbling to herself how awful it is that she can't pass by it without opening the door.
Panel 2: As she looks down at what moderns call a term I find so annoying that I want to hunt down and destroy the monster who first came up with it (i.e. "baby bump") and what I call her distended midsection and says that she's five months along and she weighs far more than she should. Her hips are gone, her seat is enormous and her ankles look like stumps.
Panel 3: She then thought-bubbles "Some people get pregnant at the front."
Panel 4: We then see a tight focus on the expression I think of when I think of Elly (a goggle-eyed
frown on a face fretting about something trivial) as she thought-bubbles that she gets pregnant all over.
Summary: If you ain't imagining her pointing to her illusory ten extra pounds and yelling at April, you don't know Elly too well. Typical Elly, right? She's going to blame April for a problem that would go away if Mike were capable of respect.
