Panel 1: For some reason, Elly is carrying a large paper bag with her as she heads out. When John asks where she's going, she tells him that she's bringing zucchini muffins over to the Enjo family.
Panel 2: He asks her why she doesn't take over the zucchini casserole instead.
Panel 3: Since she thinks that cooking casseroles is her gift to the world, she asks him if they appreciate her casseroles. He says that he does appreciate them.
Panel 4: Liz stifles a giggle when John tells an astonished Elly that while they appreciate the casserole, they are more likely to EAT the muffins.
Summary: This is only a surprise to her. After all, given Elly's issues, it's likely that she wilfully turned "It's not that we don't like your cooking. We just think you should think about what we like for once" into "We hate you, love, God, happiness and kindness" because people who love to help but are bad at it get like that. The bus garage manager on Crankshaft is the same way. She's bad at everything and overly eager to help too.
