Panel 1: Elly tries to calm Annie down by saying that while she's no expert, it seems to her that they could easily perform corrective surgery so they don't have to buy customized gloves and such.
Panel 2: Annie tells her that when Leah is older, they'll perform the surgery.
Panel 3: Since Annie doesn't want to lectured to by an idiot who she knows would be taking this far worse, she tells Elly that she can't stand this and wishes Leah was normal right now.
Summary: What amuses me is I sort of know what would happen if Mike, Liz or April had been polydactylic. Annie is reacting more or less like anyone normal would and probably resents the refugee-in-a-war-zone speech she just absorbed; Elly would unhinge her jaw and start screaming about the devil child who cruelly decided to be born with more than the proper number of fingers so s/he could make his/her poooooor mother go broke buying custom gloves.
