Panel 2: Marian the Florist tells him that he said they only had two children. John sort of confirms that.
Panel 3: She smiles as he explains that she's getting one from each child and one from him.
Summary: This is because she does something Elly is incapable of: understanding self-deprecating humor. This matches John's inability to understand who Elly is and why she might be desperate enough to accept any sign that he does despite roses saying [Timmy Turner's Dad]"Hi, honey!
