We begin the "Phil has to quit smoking" arc with his acting as if having to quit smoking is the end of the world as we know it.

Panel 1: We start things off with Phil apologizing to Mike for being late for his music lesson. It would appear that he had a doctor's appointment.
Panel 2: His telling Elly to sit down because he's got bad news...real bad news has her worried because she's thinks that he's about to tell her that he has three months or so to live.
Panel 3: Given that the terrible news that he can barely stand to say is that he has to quit smoking, her concern turns to confusion.
Summary: Given how little Elly understands about how hard it really is to quit smoking, said confusion relates as to whether dancing a jig, pointing at him and yelling "IN YOUR FACE!!!!" would be bad form.
